| Résumé : | Psychotic symptoms in Lewy body dementia (LBD) and Parkinson's disease (PD) are difficult to treat due to side effects related to antipsychotics’ antagonism of dopamine receptors. Clozapine or quetiapine are usually prescribed for this indication despite their side effect profile since they have a lower impact on motor functions. Pimavanserin is a new serotonin 5-HT2A inverse agonist indicated for Parkinson's disease psychosis without an effect on dopaminergic receptors. We present the case of a 59-year-old female patient with LBD with no psychiatric history presenting with severe delusions, visual hallucinations, and intolerance to several antipsychotic drugs including clozapine.[introduction] |
